It keeps the text as paragraphs and marks up headings where it can detect them from the PDF. Detection is a best effort, so on some files a heading may come through as normal text, or the odd break may land in a different spot. The words themselves stay intact for you to tidy up.
Not really. A scanned PDF is usually a picture of a page rather than real text, and this tool does not do OCR, which is the step that reads text out of an image. So a scan will give you little or no selectable text. For a good result, use a PDF where you can already select and copy the words.
Only in a limited way. The text inside tables and columns is usually pulled out, but the exact grid, alignment or multi-column flow may not survive, so you may need to reset it in Word. Images and complex graphics are not carried across. The focus is the words, not the layout.
